Need a consistent way to refer to quoted values in FHIR spec

      In the FHIR spec we see a mix of <code> -syle formatting (e.g. http://i.imgur.com/d4EIG1f.png), quotes (e.g. http://i.imgur.com/N0vYS0B.png), and bare words (e.g. http://i.imgur.com/r4H1iCO.png).

      The three examples above show three different ways of referring to the HTTP operation "POST". I find the first (<code>-style formatting) most readable. But in any case we should have an impose a consistent style when referring to coded terms.
