patient or another clinician.

why does it have to be constrained to report by another MD or the patient. Does that allow delivery of computer information. Receivers would not want to segregate these records by their source or kind of source. All sources (e.g. electronic, pharmacy, etc.) should be in the same resource - adjudicated appropriately to make a medication profile.
