It can't because it's about a relative, not about the patient. And very few systems would allow linking or access to data across patient charts. There is an extension that allows you to link a relative to the corresponding Patient record to allow full access to another patient's chart where this is permitted, but in that case, there'd be no need to explicitly link the Conditions to the FamilyHistory.
