      The birth time component is defined as a standard extension which contains the date and time as you have requested.

      This has been a very intentional change to attempt to prevent known issues caused by applying timezones inadvertantly/incorrectly to bith dates given their importance in many systems for identification and cross referencing.

      We will add additional notes into the description of the birthDate property to indicate that the standard extension should be used in your use case.

      Proposed Wording: birthDate dateTime

      Comment:

      birth date should be a dateTime to allow for time of birth if indicated. Particularly useful for infant care where care protocols need to be managed at greater granularoty than day. The person resource already has this element as dateTime
