Comment:

careProvider "Good Health Clinic" is omitted from the human readable narrative although it is included in the Standard Data for Patient. This does not set a good example, to leave out a piece of data that is not a "processing use only" attribute (like an OID or codesystem). I suspect this was carried over from CDA where display of "Header" is not consistently specified, and where Provider might be omitted if it was included in a Stylesheet.
