      Comment:

      Without reading the description, one would think this attribute can refer to any encounter where the condition was noted (e.g., in the case of an encounter diagnosis). Suggest renaming the attribute to clarify what it actually refers to.

      Also question why this is in a FHIR core resource. Do 80% of current systems capture this information?
