What's the use case for this extension - isn't the Provenance Resource itself supposed to be the provenance record associated with the most recent update of the resource? Or was the intention to get the Provenance Resource of the predecessor Resource to this Provenance Resource's target??? Wouldn't it make more sense to have a pointer to the Provenance Resource associated with any entity that was used in the generation of the target Resource? Then those entities' provenance records would point any entity used to generate them, so that the entire lifespan of a Resource and portions of a recource could be traced back of origination.

Replace the Provenance extension Most recent Change and add directly to the Provenance Resource as a pointer to the Provenance of each Entity involved in the current state of the target Resource.
