      FHIR extensions do have their own namespaces (in fact a full url that makes them unique). The http://hl7.org/fhir/StructureDefinition prefix is common to all extensions in the core spec, but other extensions can have their own url of choice.

      Comment:

      Extensions should be able to live in their own namespaces. Every successful, scalable, distributed system that I'm aware of uses namespaces. When I say this, I mean proper namespaces, not pseudo-namespace by naming conventions. If that possible now, it isn't documented.
