      We define extensions where we're either not sure whether 80% of systems use something or where we're certain that something doesn't make the 80% but is still used by a significant portion of systems throughout the world to define a standard way to do so.

      Comment:

      Why aren't the FHIR Core * Profiles extensions considered as core resource elements? For example, why is there a FHIR Core Goal Profile with extensions that don't existing on the core Goal resource?
