Rename the property of the careProvider on the Patient to GeneralPractitioner. (Cardinality will remain as multiple, and may be profiled to 1 if required)
Update the description of the property to include text describing this as the practitioner that is the "gatekeeper" for the patients care. Refer the implementers to standard extensions for preferredDentist and preferredPharmacist (and add these as standard extensions)

These will exist independently to values in the careTeam, which may include time boundaries, types, roles on the participants.Notification to the implementer community will be done.

Comment:
Care provider is a more complex concept than the current definition. This could represent a patient's PCP, preferred pharmacist, etc. At very least, the careProvider needs a "type" value to account for provider roles within a care team.
