      Each FamilyMemberHistory row has an identifier element that can be used to capture this. As well, there's an extension that allows a given FamilyMemberHistory to point to the family members who are mother, father, twin, etc. There's a Clinical genomics profile on FamilyMemberHistory that shows how this is done.
