It's useful to use ConceptMap to map Questionnaire questions and groups to DataElements (or their components) as well as to paths within CDA and similar artifacts. However, neither of these follow the typical "system + code" mechanism used by ConceptMap. DataElements aren't defined within a system. And for CDA, the "code" would be a path.
