      We think the logic is correct, however the expression is unclear.

      Will change the order to list "take no action" first and make the "no change" bullet last within that section

      Will change the "update" to say "all of the conditions above apply, with the exception that the value *has*changed.

      I.e. Will stop repeating the 4 bullets that are unchanged to make things clearer

      Guidelines for updating an observation and doing nothing are all the same except for the fourth one. How can you decide to update or take no action if they have the same conditions?
