Out parameters for $versions operation have error in description and incorrect type in examples

      Will correct the language "major.minor" to "publication.major".  Will change the examples to use valueCode rather than valueString.  Will define an explicit binding to a value set consisting of all of the major releases (independent of minor).  Consider drawing this from the same code system as FHIR-version, though we'd need to split into two value sets - one with the 3-level codes (used for StructureDefinition.fhirVersion) and one with the 2-level codes (used here).

      The documented type of the OUT Parameters is `code`, but both the example JSON and XML use `valueString`.

      Furthermore, their description says "Use the major.minor version like 3.0" but based on the language in http://build.fhir.org/versions.html#versions, this should be publication.major

      Associated with this, there is no specified binding for these code-type values.  One might expect it to be codes from the CodeSystem http://hl7.org/fhir/FHIR-version but this only includes codes of the form publication.major.minor
